Workers' Compensation

Worker’s Compensation covers disability incurred through accident or occupational disease—arising out of, and in the course of, employment—that requires medical, surgical, or hospital treatment.

The Diocese of Raleigh Workers’ Compensation policy is provided through Zurich Insurance. Claims are to be processed directly by the parish/school to Zurich Insurance. Claims should be reported within five days of the date of the employee’s injury. 

If an employee needs to seek treatment for a work-related injury, the employee should go to an urgent care that handles workers’ compensation cases. Please note that visits to a primary care physician for a work-related injury will not be covered by the workers’ compensation carrier. If the injury is severe, the employee should go to the emergency room of the closest hospital. An injured employee should always notify the urgent care or hospital that their injury is work related so that claims are not filed against their employer or private health insurance.

You may view the Claim Reporting Screen Shot to help you get started when submitting a claim. When filing a claim, please note there are required fields which will be indicated by an asterisk. The following information will be needed at a minimum:

Parent Company Name: Please list Diocese of Raleigh/YOUR LOCATION NAME

Policy #: WC-57-10-332

  • Injured worker and witness information
  • Nature/severity of injury
  • Lost time - if yes, first date of lost time
  • Social security number
  • Date of birth
  • Injured worker address

If for some reason you are unable to process the claim online due to technology issues or you are not at a place where you have access to file the claim online, the claim can be phoned in to the following number: 1-800-987-3373. This is only to be used at a time when you do not have the ability to file the claim online.
